If time is of the essence and you must collect your DNA
samples right away, the “Self-made Kit” option makes it
possible for you to create your own kit. Simply print the forms and instructions below, collect the
DNA samples as directed, and ship the samples and completed forms
to the address provided. When you order online or using personal
check processed by Telecheck, be sure to state “I will be
using the forms on your website” in the comments section
during checkout. We will then credit the shipping fees from your
purchase. If paying
by money order, pay only the testing fees as no shipping fees will
apply.
